DATA THAT IS COLLECTED
The personal contact and / or identification data that “CGR” will treat are name, age, sex, occupation, address, email, telephone (mobile and / or landline) and federal taxpayer registry.

In the event that the owners do not wish that “CGR” treat their personal data for the purposes set forth in paragraph b) above, they will have a period of 5 (five) days to express their refusal by directing a communication to the Department of Personal Data of “CGR” by sending an email to the following address: datospersonales@cirugiageneralriedel.com
TRANSFER
For the provision of medical-hospital services, hospitalization, surgery, nursing care, pharmaceutical services, radiology and imaging laboratory analysis, pathological studies and analyzes, therapy, rehabilitation, diet and nutrition, creation, study, analysis, updating and maintenance of clinical files, “CGR” can transfer inside and outside the country, the personal data in its possession to subcontracted third parties for the purposes indicated in this Privacy Notice. The third parties to which said data will be transferred are laboratories, clinics, hospitals, research centers, insurance companies, as well as any company of the same corporate group, or where appropriate competent authorities.
The consent of the holders will not be necessary when transfers are made to companies in the same group as “CGR” or when said transfer is necessary to provide a service or keep the clinical files updated.
In the event that the holders do not want “CGR” to transfer their personal data, they will have a period of 5 (five) days to express their refusal by directing a communication to the Department of Personal Data of “CGR” by sending an email to the following address : datospersonales@cirugiageneralriedel.com
LIMITATION OF USE AND DISCLOSURE OF PERSONAL DATA
The owners may limit the use of their personal data, through an email to datospersonales@cirugiageneralriedel.com or written notification to Domicile ——-, from Monday to Friday from 9:00 a.m. to 3:00 p.m. . and from 17:00 to 19:00 hrs., in which the limitation to the use of the desired data is indicated.
REQUEST FOR ACCESS, RECTIFICATION, CANCELLATION OR OPPOSITION OF PERSONAL DATA AND REVOCATION OF CONSENT (ARCO REQUEST)
All personal data are treated in accordance with the applicable and current legislation, therefore the holders of personal data have at all times the right to access the personal data held by “CGR” and the details of their treatment, as well as to rectify them if they are inaccurate or incomplete; cancel them when they prove to be excessive or unnecessary for the purposes that justified their obtaining; and oppose their treatment.
MEANS TO EXERCISE ARCO RIGHTS (ACCESS, RECTIFICATION, CANCELLATION AND OPPOSITION)
To initiate an ARCO Request, the owner of the personal data must submit a written request addressed to our Personal Data Department at datospersonales@cirugiageneralriedel.com or written notification to the Domicile —- service from Monday to Friday from 9:00 a.m. to 3:00 p.m. and from 5:00 p.m. to 7:00 p.m. containing the following information:
- Name of the owner and Address or email address to communicate response to request.
- Documents that prove identity or authorization to represent you in the application.
- Description of personal data on which it is intended to exercise any ARCO right.
- Date of care and name of Hospital Angeles in which you received services.
- Any other element that allows the location of personal data and attention to the request.
MEANS TO REVOKE CONSENT
